Resident
Care

Home Health Nurses provide a variety of tasks during patient visits depending on the specific plan of care and type of patient.
  • Assess and chart observations of the patient’s condition at each visit.
  • Complete evaluation tasks, including reviewing medication and vital signs.
  • Administer Physician-prescribed medication.
  • Dress or redress wounds and assess the healing progress.
  • Provide education to patients and families on proper home health care procedures and strategies.
  • Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
  • Keep the patient comfortable through palliative care.
  • Recommend tools or devices that may improve the quality of life for the patient.

Doctor
Visit at Home

· Provides direct medical services and consultation to primary care home-bound patients.

 Monitors the patient’s condition and responsiveness to the treatment provided

· Maintains complete patient records using the Electronic Medical record and complies with all reporting mechanisms

· Ensures compliance with the Department of Health Care Regulations for Licensure as well as requirements of other licensing and accrediting bodies, health maintenance organizations and other insurance carriers

· Collaborates in the provision of formal and informal education for staff

· Adheres to control and safety policies, and guidelines of regulatory agencies

· Participates in community outreach programs when appropriate

24/7 Special
Services

Our 24/7 Personal Care and Companionship programme has wide range of services :
  • Home health aides take vital signs, and they assist patients in moving from the bed to the bathroom and/ or change bedpans and garments, and they can administer basic medications. With additional education, they may operate certain medical equipment (for example, ventilators.)
  • Home health care aides may drive patients to and from medical appointments
  • Home health care aides They may assist them in purchasing groceries and preparing meals; and must be sure to follow specific dietary guidelines.
  • Home health aide may need to change bedding, assist with dressing the client and help with basic grooming.
